High smile line
Combined past root-canal therapy with post and crowns placed is showing dark margins through thin gums. However, the most serious problem is the loss of bone from chronic infection and eventual root fracture from bruxing/tooth grinding. This creates a large defect preventing restoration of healthy function and aesthetics, especially in high smile cases (that is when your gums are showing during full smile). The solution is to regain the lost bone and soft tissue as much as possible by guided bone and tissue regeneration. This process is called hard and soft tissue grafting. We augment the site to improve the base for the teeth and the gums to look normal. This is not always simple to do and results vary from patient to patient.
Bone shrinkage after tooth loss can accelerate the ageing appearance of the face. Patients with missing teeth may find that dental implants are the long-term solution. Implants involve the replacement of dental roots with titanium fixture, placed in the bone of the jaw and allowed to heal. Implant support provides a stable non-moving foundation for replacement of missing teeth. Using implants can save adjacent healthy teeth and help preserve the bone from atrophy.

The grafting procedure can be divided into two steps in conjunction with implant placement
- Simultaneous implant placement and grafting
- Initial site preparation by grafting followed by implant placement some three months later
Dr Nalbandian will consult you to determine your requirements. In most case the implant procedure and grafting is completed in one appointment.

Deficient ridge (ridge defect) due to tooth loss (traumatic extraction, past infection etc.) and denture tooth replacement. This ridge requires augmentation (grafting using soft and hard tissue) to allow harmonious aesthetically-pleasing
Implant-supported crown replacement

Three months following simultaneous ridge augmentation and implant placement. Note the improvement in ridge, shape and outline.
